You will need to complete an Application Review Form along with a copy of the original notice to suspend or cancel your accreditation. The application will only be processed when all relevant information has been provided and the application fee paid.
The Accreditation Review Panel will sit on the first Tuesday of each month, excluding January. If your appeal is lodged less than seven days before that date, your appeal will be heard in the following month. The Panel will notify you of a decision within seven days of the date of the hearing.
